Membership Benefits

By becoming a member, you agree to access the benefits as outlined in our membership program.

 

Payment Terms

You agree to pay the specified membership fee on a monthly or annual basis, as outlined in the membership agreement. Payments will be automatically deducted from your chosen payment method. If you choose the monthly payment plan, a three-month payment is required to cover the necessary testing.

 

Membership Term

Your membership term will begin upon enrollment and will continue until canceled in accordance with the terms outlined below. 

 

Use of Services

You agree to use the services provided under your membership in a lawful manner and for personal use only. Any commercial use of the services is strictly prohibited.

 

Confidentiality

We will maintain the confidentiality of your personal and medical information in accordance with HIPAA regulations and our privacy policy. 

 

Discontinuation - Cancellation of Membership

You may cancel your membership at any time by providing written notice to Apex Healthcare Institute, LLC, DBA Oasis Therapeutic Solutions. Cancellations will be effective immediately, and no refunds will be issued for any unused portion of the membership term.

Non-Payment:

Failure to pay membership fees may result in the suspension or termination of your membership.

 

Termination:

We reserve the right to terminate your membership at any time for violation of the membership agreement, including but not limited to misuse of services, safety concerns arising from noncompliance to treatment, non-payment of fees, or any other breach of terms.

 

Effect of Termination:

Upon termination of your membership, you will no longer have access to the benefits and services provided under the membership program.

 

Refunds:

No refunds will be issued for membership fees paid, regardless of the reason for termination. If you paid an annual fee, your benefits will be terminated at your membership expiration date. 

 

Agreement:

By enrolling in our membership program, you acknowledge that you have read and agree to abide by the terms and conditions outlined in this agreement. Apex Healthcare Institute, LLC DBA Oasis Therapeutic Solutions reserves the right to modify the terms of the membership agreement at any time, with or without notice, and such modifications will be effective immediately upon posting on our website or notification to members.
