

Payment Instructions
Your fee is determined by the type of services you selected when completing your online application. The non refundable initial consultation fee is $35.00, which can be deducted from your total package payment if you choose to embark on a healthier lifestyle with us.
Fees for basic services, membership and packages are subject to change.
Payment

We offer several convenient bill-pay options, debit, credit cards, and HAS and FSA payments.

We do not accept insurance payments nor gift cards.

If you choose the monthly payment plan for the virtual care membership program, a three-month payment is required to cover the necessary testing.
How to submit your payment

You can submit a one time payment or through the client portal.

You must register before you can log in to the patient portal. To register, you will receive an email from the office. The email will contain a secure link which will allow you to complete the registration process.
Once Registered and Logged on, you will be able to:

Review and pay billing statements.

Request appointments

Review personal health information.

Complete and update medical forms.

Update your profile and contact information

Scheduling Instructions
How It Works

To schedule your appointment, select your desired service and press "Book Now".

You will receive an email with required forms and consents to be filled prior to the scheduled appointment. To get the maximum of your visit, we ask that you complete all required documents prior to your visit. A booking fee of $35 dollars is required at the time of your booking which will be credited towards the total amount of payment.

If your visit requires a video visit, on the day of the appointment, you will receive a text message with the link for your virtual visit. No application download is required. You may attend the telehealth session via any computer, tablet or telephone that is camera and microphone enabled.

You will receive a confirmation text one day before and the day of your appointment. We know things happen. If you are unable to make the appointment, please cancel 12 hours before and you can always reschedule your visit.

Your credit card will be charged the total amount agreed upon between you and the provider.

Following the visit, your provider will send any prescriptions or lab orders to your respective pharmacy or lab institutions.
